To Obtain Certified Copies of Birth, Marriage or Death Certificates

The Town Clerk’s office issues certified copies of Birth, Marriage and Death records. Certified copies of Vital Records are $20 each, payable by cash or check only. Copies can be obtained in-person or by mail.

To obtain a certified copy by mail, please complete the appropriate form, and mail along with a copy of photo ID, check or money order payable to New Milford Town Clerk, and a self-addressed stamped envelope to:

Please note: Access to birth records less than 100 years old is restricted in Connecticut.  The fee is $20.00 per copy.  To be eligible to obtain a certified copy of a birth certificate a person must be:

  • The person whose birth is recorded, if over eighteen years of age
  • Such person’s children, grandchildren, spouse, parent, guardian
  • The chief executive officer of the municipality where the birth occurred, or the chief executive officer’s authorized agent
  • The local director of health for the municipality where the birth occurred or where the mother was a resident at the time of birth, or the director’s authorized agent
